Physical Well-Being at SFO

The San Francisco Health Services System (SFHSS) Well-Being Division focuses on helping members to Live, Feel and Be Better Everyday. They are offering two programs: Diabetes Prevention Program and Wellness Coaching.   

Wellness Survey  
Please take 10-minutes to complete the SFHSS Well-Being Survey by Friday, August 14, 2020. The survey is anonymous, and your feedback will help guide the development of well-being programs and identify opportunities to bring new and relevant resources to support you.  

  

Diabetes Prevention Program 
Participate in this 12-month virtual program to help you make lifestyle changes to lose weight, improve health, and reduce your risk for type 2 diabetes. Join us for an informational session and meet a Diabetes Prevention Program Lifestyle Coach on Wednesday, August 12, 2020 at 11:00am

  

Program Start Dates: 

  • Option 1: Program starts September 15th and meets Tuesdays from 3pm to 4pm. 
  • Option 2: Program starts September 17th and meets Thursdays from 5pm to 6pm.

Virtual Wellness Coaching 
Get the support you need with tips, tools, and resources that can help you reach your goals. Create healthy habits and stay motivated in your wellness routine with the support from a Wellness Coach.
